You can obtain a Standard (including Student Options), Gold, Platinum or Golf Credit Card application form, Upgrade / Transfer your Credit Card and Credit Limit Increase form from here.

The forms are in Adobe Acrobat format, which means that you will have to download the Adobe Acrobat Reader software if you don't already have it.

You can also request an application form be mailed to you, or call us on 13 2221, 8am to 8pm, Monday to Friday, to apply.

To obtain, complete and lodge your Credit Card Application simply complete the following steps:

1. Download the
      Standard (including Student Options) Credit Card application form
      Gold Credit Card application form
      Platinum Credit Card application form
      Golf Credit Card application form
      Upgrade/Transfer your existing Commonwealth Bank Credit Card
      application form
      Credit Limit Increase application form.
2. Print the application form. To ensure the form prints correctly, please select "Shrink to Fit" from the File/Print menu option.
3. Complete the application form in full. If you are not currently a Commonwealth Bank customer you must provide satisfactory identification. You will need to: 

   •	Download and print the Certified Copies Identification Form; or
   •	Go to your nearest branch for an identification check.

4. Fax the completed Standard, Gold or Golf Credit Card application to us on (02) 9635 4200, or take it to any Commonwealth Bank branch, where it will be forwarded to our Credit Card area for processing.
   
   If you are not currently a Commonwealth Bank customer you will need to mail your application with your certified identification documents to: Reply Paid 133, The Manager, Credit Assessment, PO Box 9823, Parramatta NSW 2124 (no postage stamp required).
