How to request your American Express
Card
If you’re an existing Commonwealth Awards cardholder, you don’t need to
apply for your Commonwealth Bank American Express Card, you just need to ‘opt
in’ to receive it.
You can do this in one of four ways:

Request an American Express Card within NetBank
- If you are not registered with NetBank, you can complete the online request form
- Call 13 2221, 24 hours, 7 days a week and we’ll send you a fulfillment kit,
which includes a request form that you must sign and complete
- Visit any branch to complete the registration form.
