If your smoking status changes, it is important you tell us as it may affect your premium and the acceptance of any claims that may occur.

If you have stopped smoking for at least 12 months you can apply for non-smoking premium rates which may reduce your premiums.

Just call 13 1056 between 8am and 8pm (Sydney time) Monday to Friday to get a quote. A Confidential Non-Smoker’s Declaration Life and Disability form will be sent to you to complete.

Alternatively you can complete the Confidential Non-Smoker’s Declaration Life and Disability form.

  • return it by email to cservice@cba.com.au

  • post to PO Box 319 Silverwater NSW 2128; or

  • fax to 1800 258 828
